Jacob Danial Kwiatkowski

Jacob Danial Kwiatkowski, 18, of Delta, OH, passed away Sunday, November 20, 2016. He was born in Toledo to Chad D. and Cherrie B. (Saam) Kwiatkowski on July 21, 1998. He graduated from Delta High School in 2016; prior to that he attended Clay High School in Oregon, OH. Jacob loved playing video games, listening to music, and spending time with his family and friends; especially his long-time friend, Sara Turner. Although Jacob struggled, he was a good hearted young man that fought hard against addiction. Organizations that helped to support Jacob’s battle include Synergy and Recovery Services. Along with support groups, Jacob also proudly accepted God into his life. Jacob had many mentors in his life, including Marty Friess, his retired high school principal, who helped him with job skills and training. Jacob is survived by his loving family; father, Chad (Erin McPartland); mother, Cherrie (Chris) Rayburn; step-sister, Miranda Rayburn; half-sister, Riley Rayburn; grandparents, Richard and Carol Kwiatkowski, Pam and Don Saam; great-grandmother, Bernice Saam; and many aunts, uncles, cousins and close friends. Friends will be received on Sunday, November 27, 2016 from 2:00 P.M. to 8:00 P.M. at Witzler-Shank Funeral Home, 222 E. South Boundary Street, Perrysburg (419-874-3133). A Graveside Service will take place on Monday, November 28, 2016 beginning at 10:00 A.M. at Fort Meigs Union Cemetery, 620 W. Indiana Ave, Perrysburg.
