David E. Crouch, Sr., M.D., 71, of Port Clinton, died in his home, Thursday afternoon, May 14, 2015, surrounded by his family. David was born January 5, 1944, in Dayton, Ohio, to Joe T. and Cleo (Munday) Crouch. On March 10, 1964, in Winston Salem, North Carolina, he married Phyllis M. Simpson and she survives. He received his bachelors degree from the Ohio State University and in 1970 received his medical degree from OSU. Dr. Crouch held a family practice since 1971 and retired in 2005. His memberships included the Elks, the Moose and the Colonial Club, all in Port Clinton. Davids hobbies were collecting antiques, golfing, fishing, hunting and woodworking. He was a fan of NASCAR and the Ohio State Buckeyes. His most special enjoyment was spending time with his family. In addition to his wife, Phyllis, David is survived by his sons, J.T. Crouch and David (Kelly) Crouch, Jr., his grandchildren, Andrea Murray, Jesse, Paige and Abby Crouch, and his sister, Jo (Bill) Coon, as well as four nephews. His parents preceded him in death. Visitation will be at the Robinson-Walker Funeral Home & Crematory, 165 E. Water St., Oak Harbor, Monday, May 18, 2015, from 2-8 p.m., with the family present from 2-4 and 6-8 p.m. The funeral service will be conducted 11:00 a.m., Tuesday, May 19, 2015, at St. Pauls Lutheran Church, 9789 W. Oak Harbor SE Rd., Oak Harbor, OH 43449. Interment will follow in Salem Township Union Cemetery, Oak Harbor. In lieu of flowers, the family has suggested memorials for David to be directed to St. Pauls Lutheran Church, Oak Harbor.
